What All-on-4 actually is
Four implants holding one fixed arch — not a denture
The name describes the engineering: an entire arch of teeth, supported by four implants. The two front implants go in straight; the two back implants go in at an angle, which is the whole trick — angling them lets us anchor into the denser bone toward the front of the jaw and skip the thin, resorbed bone at the back that would otherwise demand grafting or a sinus lift. Fewer implants, less surgery, and a case that finishes for people who were told for years they were not implant candidates.
It stays in
The bridge is fixed to the implants. You do not take it out, you do not soak it overnight, and there is no adhesive involved. You brush and water-floss it the way you would natural teeth, and we check underneath it at your regular cleanings.
It bites like teeth
A conventional denture returns a fraction of natural biting force and moves against the gum while you chew. Because the arch is anchored in bone, All-on-4 restores close to natural function — which in practice means apples, steak, and corn are back on the table after healing.

What the All-on-4 process looks like
One surgical day, then four to five months of quiet healing
The sequence below is the standard full-arch path in our office. Every step happens here — planning, surgery, and the final bridge — so you are not handed off between an oral surgeon across town and a restoring dentist who never saw the plan.
CBCT scan & written plan
A 3D cone-beam scan shows exactly how much bone you have and where it is dense enough to anchor. You see your own scan on screen, we map the implant positions against it, and you leave with the all-in price for the arch in writing. The scan is free.
Surgical day — you leave with teeth
Any failing teeth come out, four implants go in, and a fixed temporary bridge is attached the same day. Sedation is available and most patients want it. Plan to rest at home for about 48 hours afterward — schedule it against a long weekend.
Integration & soft-diet window
The implants fuse to bone while the temporary bridge stays in place — you are never without teeth. Soft foods for the first 8–10 weeks is the one rule that matters; it is what protects the implants during the fragile early weeks.
Permanent bridge delivered
The final fixed bridge is fabricated to the shape and shade you approved, then seated and adjusted. From here it is regular cleanings and a check under the arch — the case closes at roughly the five-to-six-month mark.
Is All-on-4 right for you?
Clear candidates — and when we say something else first
Full-arch is a big decision and it is not the answer for everyone who walks in asking about it. Where it is the obvious call, and where we will point you somewhere better:
- Best for: an arch where most teeth are failing or already gone, advanced gum disease loosening a whole upper or lower, or a removable denture that slips, clicks, and has quietly narrowed what you are willing to eat.
- If you still have healthy teeth, we save them. Extracting sound teeth to fit a full-arch case is the wrong trade. When only part of the arch is failing, individual implants or a bridge usually cost less and keep more of your own structure.
- Uncontrolled diabetes and smoking change the math. Both slow healing and raise the risk that an implant does not integrate. We do not refuse to treat — we plan around it, coordinate with your physician where that helps, and tell you plainly what your odds look like before you commit.
- Severe bone loss may still need grafting. Angled implants avoid most grafting, not all of it. If the CBCT shows there is not enough bone even at the front, we will say so at the consult rather than discover it on the surgical day.

Do I really leave with teeth the same day?
Yes — that is the defining feature of All-on-4. On the surgical day (about 4–6 hours, sedation available) we remove any failing teeth, place four implants at angles that use your existing bone, and attach a fixed temporary bridge before you go home. You do not leave with a gap and you do not leave with something removable. What you leave with is a provisional, not the final: you eat soft foods for the first 8–10 weeks while the implants integrate, then we deliver the permanent bridge at about the 4–5 month mark.
Am I a candidate if I have been told I do not have enough bone?
Very often, yes — that is the specific problem All-on-4 was designed around. The two back implants are placed at an angle so they anchor into the denser bone toward the front of the jaw, which lets us avoid the thin or resorbed areas at the back that would otherwise require a sinus lift or major grafting. Patients told years ago that they "aren't an implant candidate" were usually being evaluated for one implant per missing tooth, which carries a much higher bone requirement. The 3D CBCT scan settles it in about ten minutes and it is free at the consult — we will show you your own bone on screen and tell you honestly if grafting is still needed first.
All-on-4 or a snap-in implant denture — which should I choose?
The honest fork is fixed versus removable. All-on-4 is a fixed bridge: it stays in your mouth, you brush it like teeth, and it restores close to natural biting force — that is the choice if you want to stop thinking about your teeth. An implant-supported overdenture snaps onto two to four implants, costs meaningfully less, and eliminates the slipping and adhesive of a conventional denture, but you still take it out at night and it does not chew quite as hard. How long does the whole All-on-4 process take, start to finish?
About 5 to 6 months from consult to final bridge. The consult and planning visit runs roughly 90 minutes, the surgical day is 4–6 hours, healing and integration take 4–5 months with the temporary bridge in place the whole time, and the permanent bridge is delivered over one or two short visits. The part patients underestimate is the recovery immediately after surgery: plan on resting at home for about 48 hours, which is why most of our patients schedule the surgical day against a long weekend.
